If you would like to become a patient of Lighthouse Hospice Group (or if you are the legal representative of patient), please complete the Care Inquiry or Authorization to Disclose Health Information form.  If you are the legal representative (POA) of said patient, please submit the form and POA documentation to Lighthouse Hospice Group via fax (803.781.1936) or e-mail.
